Important
Dates and Policies
Your registration must be received by October 14, 2005 to receive the
Early-bird rate. Conference fees must be paid in full and accompany a
completed registration form, either online or a printed version.
If you must cancel for any reason, please notify us in writing by October 14,
2005 in order to receive a refund, less a $150 administrative fee. No refunds
will be issued after October 14, 2005.
If you’d like to make changes, updates or substitutions to your registration,
you can do so at any time by calling 800.543.0874, ext. 2205. You will be
required to provide your Confirmation number and first and last name, exactly
as it was entered when registering.

address. Registration confirmation and program updates will be communicated
through email.
You should receive an email confirmation immediately following your
submission. If you don’t receive one, please give us a call at 800.543.0874
and we’ll send it immediately upon request.

We’ve made sure the facility we are in has total accessibility, and we will do
whatever is needed to make attending the Conference as convenient as possible
for all attendees. If you have a special need, please let us know on when
registering. We will contact you before the Conference to make arrangements,
if necessary.
